Patient Access

ALL patients including children have been allocated a named, accountable GP.  This named GP is allocated when first registering with the practice but for those patients who have been registered  with us for some time you can find out the name of your named and accountable GP by asking at reception.

This online access allows patients or their carer’s, where appropriate, to view their GP medical record securely via the internet. Patients will be able to view their medical history including medications, vaccinations and any allergies recorded.

A patient information leaflet is viewable below. If you would like to register for this new service please download the appropriate consent form (patient, carer or parent) or alternatively ask in reception for the consent form. Once completed return the consent form to us with some form of photo ID e.g. passport, driving licence etc., when we will then issue you with a registration letter to create a Patient Access account.
