Registration Process

If you live within the practice area and wish to register with the surgery, you will need to complete a registration form.

Registration forms can be collected from the surgery in person or can be downloaded below.

Once these forms have been filled in please return them to us at the surgery. You will need to bring ID along with you to enable us to register you - please see the registration form for what type of ID is required.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records from your previous GP practice.
